Required Athletic Forms for TCS Student-Athletes

At Trinity Christian School Athletics in Sharpsburg, GA, completing your child’s athletic forms early ensures a smooth start to the sports season. We prioritize safety, compliance, and clear communication so your Lions athlete can focus on training, games, and team growth.

Below you’ll find all necessary forms, procedures, and checklists—along with efficient steps to get it done right the first time.

Step 1: Review Physical & Medical Forms

All student-athletes (grades 5–12) must submit a current physical form and medical history questionnaire before tryouts or practice.
Where to get them:
- TCS Wellness Portal via Rank One (current TCS digital system)
- Or print the physical form from the TCS portal linked below
Age & Frequency: Must be completed by a licensed provider within 365 days of the first athletic event each school year.

Step 2: Complete Health & Liability Waivers

Forms include Physical (4 pages), Concussion Awareness (online), Sudden Cardiac Arrest Awareness (online), and Athlete/Parent Handbook (online)
These ensure students meet GHSA guidelines and that TCS Athletics can support every child in case of injury.
How to finish fast:

Download all waivers at once.
Fill them out electronically.
Keep a copy for your records.

Step 3: Submit Your Forms

Digital submission: Upload the forms directly to Rank One.
Confirmation: Expect an email confirmation from Rank One once your athlete is cleared. Reach out to our athletic department if you need help by emailing athletics@tcslions.org.
